 What does UPAT do for teen parents?

•       Provide free bi-monthly home visits

•       Help teens through crisis, by helping to provide basic needs and referrals to community resources

•       Offer Infant and toddler parenting and developmental information

•       Provide Parent child educational activities

•       Present monthly teen group connections meetings to promote social interaction with other teen parents

Program Benchmarks

•       Improved maternal and newborn health

•       Reduction in child injuries, child abuse, neglect and maltreatment, reduction of emergency department visits

•       Improvements in school readiness and achievement

•       Domestic  Violence prevention

•       Coordination and referrals for  community resources

•       Family Economic and Self Sufficiency
